ADAP Enrollment

HIV/AIDS

To enroll in Florida ADAP, follow these four steps:

1. Find out if you qualify.

Before you can be enrolled in ADAP, you must be determined eligible to receive HIV Patient Care Services through the Patient Care Eligibility screening process.

You can fill out some of the forms online (When submitting electronic forms, you understand that these forms may be received by the Florida Department of Health and/or lead agencies contracted with the Florida Department of Health to provide case management, ADAP and eligibility services as well as their subcontractors.)

Contact your local ADAP Office for more information. Once you have been determined eligible, please be sure to bring the Eligibility Letter with you to the ADAP Office for enrollment.

The HIV/AIDS Patient Care Eligibility Process will request and review the following:

  • Proof of HIV+ status
  • Proof of living in Florida
  • Insurance screening: Medicaid, Medicare, private and others
  • Proof of income (400% of the Federal Poverty Level)

2. Schedule your ADAP appointment.

3. Enroll in ADAP.

Once you have been determined eligible (Step 1) and scheduled your ADAP appointment (Step 2), you can move forward with ADAP enrollment.

What do you need for ADAP Enrollment?

  • A Patient Care Core Eligibility Form
  • A prescription for at least one antiretroviral
  • Insurance documentation, if applicable. See the ADAP Insurance tab for more information on insurance.

4. Get your prescriptions filled.

  • Uninsured clients in a county health department that has no onsite pharmacy will be served through a specialty pharmacy – please visit our Specialty Pharmacy Services page.
  • Clients with insurance will pick up medications from a CVS retail store or a participating pharmacy.

A valid form of identification will be needed in order to pick up prescriptions. Make sure you take your medications as prescribed by your doctor. For more information about medications the program carries, please refer to the ADAP Formulary.

IMPORTANT

If you do not qualify for ADAP, contact your case manager. You may qualify for Partnership for Prescription Assistance (PPA) and for patient assistance programs (PAPs). There are over 180 programs offered by pharmaceutical companies that provide free or low-cost drugs for individuals who cannot afford to pay for them. Each program has specific eligibility criteria and coverage.
