Insulin Distribution Program

The Insulin Distribution Program is a safety-net program and is only to be used when there is no other resource available for the insulin.

Central Pharmacy and county health departments (CHDs) with a licensed pharmacy are authorized to fill client-specific insulin prescriptions. CHDs without a licensed pharmacy will forward insulin prescriptions to the Central Pharmacy.


Eligibility Determination and Documentation

Individuals applying to participate in the program must complete the program application and provide a current prescription for insulin. The application is available in English, Spanish, and Creole.

When the completes application and prescription for insulin is received, the CHD will determine the applicant’s eligibility based on the criteria below:

  • Self-declared resident of Florida
  • Diagnosis of diabetes
  • Uninsured, lacking insurance that covers insulin, or has an insurance deductible or copay that the applicant cannot afford
  • Net family income at or below 100% of the federal poverty guidelines
  • Has no more than $2,500 in private funds, bank accounts, or assets, other than a homestead
  • Not a current Medicaid recipient

The CHD has the authority to make the final determinations of eligibility for the Insulin Distribution Program.
