What is the Annual Report?
The Annual report is a comprehensive review of data concerning significant events and transfers during prenatal, intrapartum and postpartum care provided by licensees in Florida, as well as major activities of the Council, rule recommendations, Council meetings, and other information pertaining to the profession. Annual reporting from individual midwives is required per 64B24-7.014(6), Florida Administrative Code, which states in part:
Each licensed midwife, temporary certificate holding midwife, and midwife supervising a student midwife in assisting in childbirth that occurs in an out-of-hospital setting, shall file an annual report no later than July 31 for the prior fiscal year...
Submitting Annual Report Data
- Complete the required form (DH-MQA 5011, Annual Report of Midwifery Practice).
- Submit the required form to MQA.MidwiferyAnnualReport@flhealth.gov no later than July 31st.
- Questions and concerns about the annual report should be directed to MQA.Midwifery@flhealth.gov.
Failure to submit the Annual Report of Midwifery Practice will result in a notice of noncompliance issued by the Department.
