The practitioner profile summarizes data submitted to the Division of Medical Quality Assurance by allopathic, osteopathic, chiropractic, and podiatric physicians and advanced registered nurse practitioners as required by law. The practitioner profile summarizes data submitted by the practitioner that has not been verified by the Department unless otherwise indicated. The practitioner is responsible for updating the profile and adding new information to the profile. Thus, neither the Department of Health nor the State of Florida accepts any legal liability or responsibility for the accuracy, timeliness, completeness, or usefulness of this information.
The department verifies criminal history by conducting statewide criminal background checks. The department also verifies disciplinary actions and liability claims against the practitioner by conducting a search of the National Practitioner Data Bank. The practitioner is required to list any disciplinary action or liability claim that has occurred in the last ten years. Therefore, the practitioner may have disciplinary action and liability claims that occurred more than 10 years ago that are not published on the practitioner profile. Additionally, federal criminal history information is not available to the public and thus, is not listed here.
