Chronic Conditions/Disease Management Program
Some people have an illness that can get worse if not watched closely by nurses and doctors. Your Nurse Care Coordinator is here to help. Call your CMS Area Office for more help. We have special, free programs for some illnesses. This is called Chronic Conditions/Disease Management. These programs help you manage your care. They help you stay well and not get worse. We have programs for diabetes, asthma, sickle cell anemia and ADHD/ADD. You can ask your PCP for more information or call your care coordinator.- Eligibility

Infants, children, and adolescents under age 21 who are enrolled in the CMS Plan and have been diagnosed with diabetes, asthma, sickle cell anemia or ADHD/ADD are eligible for services provided by this program. To be enrolled in the CMS Plan, children must meet the clinical and financial eligibility criteria mandated by Florida Statutes.
To find out if your child is eligible to receive services from the Chronic Conditions/Disease Management Program, contact your child's primary care provider, or your local CMS office.
Healthy Behaviors Program
Healthy Behaviors is a program that encourages behaviors designed to improve the enrollee's overall health. All enrollees will be informed, once enrolled, about the CMS healthy behaviors program. All enrollees are eligible, if interested. CMS offers Health Behaviors Programs for the following three (3) conditions:
- Smoking Cessation to stop using tobacco
- Healthiest Weight to lose weight
- Alcohol or Drug Abuse recovery program
