Chronic Disease Prevention Workshop

The goals of the WORKSHOP are to:

• Provide technical assistance to chronic disease programs and staff at county health departments and other partnering organizations on state and federal program policy, data to action, client and partner engagement, program sustainability, and evaluation.
• Strengthen program staff and partners knowledge of program resources and services delivery skills for diabetes, heart disease, and stroke prevention and management.
• Technical assistance will include providing tools and strategies to support policy, data to action activities for statewide and local chronic disease prevention programs, and partner services such as dental, tobacco prevention, and rural access.
• Bring awareness of gaps to chronic disease health services and promote strategies and tools to mitigate these gaps.
• Provide information on state and county wide chronic disease partnerships and services such as tobacco and dental that can be leveraged to increase services to target population and in rural communities.

Audience
The WORKSHOP target audience is chronic disease prevention program staff at county health departments and community partners.
