Minority Health Liaisons 

Minority Health Liaisons, designated in each county, support the Office of Minority Health in improving health outcomes of racial and ethnic minorities and other vulnerable populations in communities.

These liaisons engage in partnership engagement, minority health planning efforts, and implementation of projects to improve social determinants of health.

Minority Health Plans 

This plan will guide efforts to improve and create strategies to address systems and increase opportunities to achieve health. A goal of this plan is to collaborate with local partners and utilize county-level data.
