The Telehealth Minority Maternity Care Program (Telehealth Program) offers prenatal and postpartum services and care for women throughout their pregnancy and up to one year after birth at no cost. Enrolled participants have access to comprehensive care and continued support from enrollment through the post-partum period.

Telehealth helps reduce barriers to care and compliments traditional in-person medical visits which are essential for performing certain tests and physical examinations.

Services provided by the Telehealth Program include but are not limited to:

  • Screening and treatment for common pregnancy related complications.
  • Referrals and connection to care and wrap-around services.
  • Connection to medical devices (such as scales and glucose monitors).
  • Health literacy information and education.

Screening and treatment of common pregnancy related complications include but are not limited to:

  • Anxiety
  • Cardiovascular conditions
  • Cerebrovascular accidents
  • Depression
  • Diabetes
  • Hemorrhage
  • Hypertensive disorders relating to pregnancy
  • Infection
  • Substance use disorder

Connection to wrap-around services include but is not limited to:

  • Housing placement options
  • Transportation services
  • Home visitation/education
  • Lactation support
  • Nutrition counseling
  • Car seat installation and training
  • Wellness and stress management
